How it works
By providing high-quality personal care and emotional support, our team aims to enable patients to remain being cared for in their own home, surrounded by those who are important to them. Our care also considers all stages of bereavement, including visits and pastoral support where necessary from our team.
Who can access the service?
Patients who are registered with a GP in Nuneaton, Bedworth or North Warwickshire, and who have a palliative condition or require end of life support.
How do I access the service?
Your GP, District Nurse, or other Health Care Professional can refer you to the service by calling 024 7686 5441 between 8.00am – 8.00pm, 7 days a week. Alternatively, you can always call us yourselves and we will talk with you about how to proceed.
Care: How can we help?
The care we provide is individual to you. On our first visit to your home, a nurse will spend time talking with you. This allows us to identify ways in which the Hospice at Home team can most benefit you and your family/carers. In addition to helping you directly with personal care, we also work closely with your District Nurse and GP. We may request permission to contact them to discuss your care.
Our support can include: Personal care, toileting, pressure area care, emotional and spiritual care.
Confidentiality
To ensure that you always receive the care and support that is right for you, we have to ask you for personal information. We keep this information together with details of your care, because it may be needed if we ever have to see you again. We may use this information for other reasons, for example; Checking the quality of care we provide and planning our services for the future. Whenever possible, we remove details that identify you. Everyone working at Mary Ann Evans Hospice has a duty of confidentiality. We only use, or pass on information about you if people have a genuine need for it, and if it is in yours and everyone’s interest. You are entitled to have access to your health records. Please ask a member of hospice staff for further information, or if you wish to discuss the contents of your records.
Comments, compliments and complaints
We are always pleased to receive your comments or concerns about our services, compliments are always appreciated and can be given to us verbally or in writing. If you are concerned about any aspect of your care, please don’t delay in speaking with a member of staff who will do their utmost to help you. If you remain concerned or need further help a member of staff can advise you of our complaints procedure.
